Fixed Term for 12 months (may be extended subject to additional funding)

Our team of Personal Support Navigators is expanding and we are recruiting for 2 additional roles, to provide services in the following locations

• Bradford South (Tong)
• Bradford East

The Team helps people who are living with long-term conditions and works with them and other organisations to co-design and co-produce an innovative combination of support to achieve the goals that the person identifies as most important to them.

We are looking for people who ideally have experience of managing a caseload of individuals with varying needs and issues, successfully supporting people to achieve their goals and positive outcomes. Experience of partnership working and engaging with organisations and professionals is also desirable.

You will be committed to using a person centred approach, ensuring that the personal aims and choices of the people we work are represented. 

We encourage agile working and people may be supported either over the phone, via video link, in the Community Hubs or through home visits.  

The nature of the post will require travel across the District. A full driving license, appropriate insurance cover and use of a car is essential.

Salary £23,230 per annum

37 hours per week Monday – Friday, although occasional evening or weekend work may be required

5 weeks holiday plus bank holidays pro rata
